Welcome to Flowerstone Family Health Clinic
We are a Nurse Practitioner led Patient Medical Home, located in Qualicum Beach, BC. Our goal is to deliver comprehensive team-based primary health care to individuals and families in the Oceanside Area.
This clinic is a part of the Oceanside Primary Care Network, and in the upcoming months will be welcoming other clinicians to support the provision of care, such as Physicians, First Nations Knowledge Keepers, nurses, mental health and substance use clinicians, social workers and pharmacists. Flowerstone Family Health Clinic is your medical home and each patient will have a primary provider who knows their history, will listen to their story, and provide expertise that allows them to participate as active partners in their care.
A nurse practitioner (NP) is a registered health professional with a graduate level university education who can provide a broad range of primary health care services that are individual for you. Once you are attached to a NP at Flowerstone Family Health Clinic, a NP becomes your primary care provider and will work with you to meet your health needs across your lifespan.
COVID-19 Precautions
We are offering phone and virtual video appointments.
Other initiatives: prescription renewals, review of test results, consultations and follow ups can be conducted by phone or virtual video. Any in-office visit that is essential will be arranged by the provider at the time of your telephone or virtual visit.
If you have a clinic appointment, please arrive on time. If you are early, we request you wait outside or in your car until your appointment time.
When your nurse practitioner determines you need to be seen, please keep the following in mind:
- We would like you to wear a mask
- Patients will be asked to use hand sanitizer
- We will ask your history to rule out any COVID symptoms
- To ensure social distancing waiting area chairs will be spaced apart
- Just bring what you need, do not bring backpacks, carts or other items
- We would ask only the patient arrive for the appointment, unless accompanying a child or someone with disabilities
